Home Health Licensed Practical Nurse in Mount Washington, KY

Deliver skilled and compassionate nursing care to patients where they need it mosttheir homes. As a Licensed Practical Nurse specializing in home health, you'll ensure that each patient receives individualized medical attention and ongoing support tailored to their needs. Your nursing expertise will help patients gain independence and improve their quality of life, all while communicating clearly with families and collaborating with an interdisciplinary healthcare team.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Occasional flexibility based on patient needs

Qualifications:

• Active and unrestricted LPN license in the state of practice
• At least one year of experience in home health, hospice, or a similar community-based setting
• Proficiency in using Home Care Home Base (HCHB) for charting
• Current BLS (Basic Life Support) certification
• Valid drivers license, reliable vehicle, and current insurance
• Ability to work independently and manage a diverse field-based caseload
• Strong clinical judgment, time management, and communication skills
• Preferred: Medicare-certified home health experience and an in-depth understanding of compliance standards

Key Responsibilities:

• Conduct skilled nursing assessments and develop/update patient-centric care plans
• Administer medications, wound care, IV therapy, and additional clinical interventions as ordered
• Monitor and report patient condition changes to the RN Case Manager or physician
• Educate patients and caregivers on chronic disease management, medication adherence, and safe self-care
• Accurately document all care and communication in Home Care Home Base (HCHB)
• Collaborate with fellow clinicians and participate in interdisciplinary case conferences
• Uphold infection control best practices and rigorous safety standards in the home environment
