Home Health Registered Nurse (RN) Pine Hill, Alabama

An experienced and compassionate Registered Nurse is needed to provide skilled home health care to patients. This contract opportunity offers the chance to make a significant impact in patients lives by delivering high-quality, patient-centered nursing care in the comfort of their homes. Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct OASIS assessments and develop individualized plans of care.
• Provide a range of skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient status, respond promptly to changes in condition, and coordinate care with physicians and other providers.
• Offer patient and family education on treatment plans, medications, and health management.
• Complete accurate and timely charting using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides for cohesive care delivery.
• Participate in case conferences and ongoing staff development.
• Comply with regulatory requirements and agency guidelines.

Qualifications:

• Active RN license in good standing
• At least one year of recent home health nursing experience in the field
• Proficient in using Home Care Home Base (HCHB)
• Demonstrated OASIS assessment skills
• Reliable transportation, valid drivers license, and auto insurance
• Strong clinical assessment, time management, and communication skills
• Ability to work independently in a field-based role
• BSN preferred; experience managing 2532 points weekly and familiarity with Medicare compliance standards is a plus
