Home Health Registered Nurse in Franklin, KY

An outstanding opportunity is available for an experienced Home Health Registered Nurse (RN) to deliver skilled nursing care directly to patients in a home setting. This full-time, 13-week contract involves providing high-quality, patient-centered care while working with autonomy and professionalism.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop tailored care plans for each patient
• Deliver hands-on skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor and evaluate patient progress, recognize any significant changes, and communicate updates to the care team and physicians
• Educate patients and families about conditions, medications, treatment protocols, and preventive health strategies
• Accurately document all care and updates in Home Care Home Base (HCHB) system
• Collaborate with a multidisciplinary teamincluding physical and occupational therapists, social workers, and home health aidesfor coordinated patient support
• Attend case conferences, staff meetings, and participate in ongoing professional development
• Adhere to all regulatory, compliance, and documentation requirements

Required Qualifications:

• Active RN License in the state of practice, in good standing
• At least 1 year of recent Home Health field experience
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Excellent clinical assessment, critical thinking, and organizational skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to thrive independently and manage a flexible field-based caseload

Preferred Background:

• BSN preferred
• Experience handling 2532 points per week caseload
• Familiarity with Medicare and home health compliance standards
