Home Health Licensed Practical Nurse in Louisville, KY

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional flexibility to accommodate patient needs

Key Responsibilities:

• Complete skilled home visits to assess, implement, and monitor individualized care plans
• Administer medications, manage wound care, and coordinate IV therapies as ordered
• Observe and report any changes in patient conditions to the RN Case Manager or physician
• Educate patients and caregivers on illness management, medications, and effective self-care
• Document clinical care and communication accurately using Home Care Home Base (HCHB)
• Collaborate closely with the interdisciplinary team to ensure the highest standards of care
• Uphold infection control protocols and safety at every visit
• Attend case conferences, contribute to quality improvement initiatives, and stay current with required training

Essential Qualifications:

• Active, unrestricted LPN license within the state of practice
• At least one year of experience in home health, hospice, or community-based nursing care
• Proficient with Home Care Home Base (HCHB) electronic charting
• Valid drivers license, reliable transportation, and current vehicle insurance
• Current BLS certification
• Strong clinical assessment, communication, and organizational skills
• Ability to work independently while meeting the demands of a field-based caseload

Preferred Experience:

• Familiarity with Medicare-certified home health agencies
• In-depth knowledge of home health compliance and federal/state documentation standards
• Demonstrated excellence in interpersonal communication and patient advocacy
