Licensed Practical Nurse Home Health Maumelle, Arkansas

Experienced and compassionate nurses passionate about delivering care in the home setting are invited to apply for this Home Health Licensed Practical Nurse (LPN) opportunity. This contract role is ideal for a skilled LPN with a strong background in home health, looking to provide high-quality, patient-centered care and to support recovery and independence for a diverse patient population.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Occasional flexibility to accommodate patient needs
• Field-based role visiting patients within an assigned territory
• Workload: 2532 points per week (equates to approximately 35 patient visits daily)

Essential Qualifications:

• Active, unrestricted LPN license in the state of practice
• At least one year of recent experience in home health, hospice, or a similar community-based role
• Proficiency using Home Care Home Base (HCHB) for documentation
• Current BLS (Basic Life Support) certification
• Valid drivers license, insurance, and reliable vehicle
• Excellent clinical judgment and time management skills
• Ability to work independently and manage a dynamic patient caseload

Preferred:

• Previous experience in Medicare-certified home health agencies
• Familiarity with compliance and documentation standards
• Strong interpersonal and patient education abilities

Responsibilities:

• Make in-home skilled nursing visits to assess, plan, and implement patient care plans
• Administer medications, perform wound care, and manage IV therapies as ordered
• Monitor and report changes in patient condition to the care team
• Educate patients and caregivers on disease management and health maintenance
• Document all assessments and interventions using HCHB
• Coordinate care with the interdisciplinary team and participate in quality improvement activities
