Job Summary:

TheraCare Home Health is seeking a dependable and patient-focused Licensed Vocational Nurse (LVN) to provide skilled nursing care in the home setting under the supervision of a Registered Nurse. In this role, you will support patient care delivery, assist with care planning, and collaborate with the interdisciplinary team to ensure high-quality outcomes. QualificationsQualifications:

Licensed Vocational Nurse (LVN) in the state of Texas

Minimum of 1 year of home health experience preferred

Strong knowledge of patient care, infection control, and documentation practices

Ability to follow and implement a Plan of Care (POC) under RN supervision

Strong clinical assessment, documentation, and communication skills

Ability to identify and report changes in patient condition

Experience working with interdisciplinary teams in a home health setting preferred

Reliable transportation with valid driver’s license and current auto liability insurance

Ability to read, write, and communicate effectively in English

Essential Functions:

Provide skilled nursing care to patients in the home setting under the supervision of a Registered Nurse

Assist in identifying patient physical, psychosocial, and environmental needs

Participate in developing and implementing the Plan of Care (POC) in collaboration with the RN

Administer treatments and interventions in accordance with physician orders and agency policies

Monitor patient condition and report changes to the supervising RN

Maintain accurate and timely clinical documentation in compliance with regulatory standards

Participate in interdisciplinary communication and weekly case conferences (IDG/IDT)

Provide verbal and written updates to the RN prior to reassessment, supervisory, and discharge visits

Follow and maintain infection control and safety protocols (OSHA standards)

Ensure patient safety and quality of care at all times

Comply with all state regulations and professional standards of nursing practice

Participate in required in-service training and maintain clinical competency

Meet productivity expectations and ensure documentation is completed within required timeframes
