Job Title: Call Center Triage Nurse LPN/RN
Pay Rate-$70,861-$97,405 (Compensation will be determined based on qualifications and experience)
About the Role:
The Call Center Triage Nurse is responsible for managing patient triage calls, coordinating urgent appointments, supporting clinical communication, and providing nursing guidance within the call center environment. This role requires strong clinical judgment, excellent communication skills, and the ability to accurately assess patient needs, document information, route calls appropriately, and deliver exceptional customer service while collaborating with providers, staff, and community healthcare partners.
Minimum Qualifications:
- Required -High School graduate or GED certificate and completion of an accredited nursing program.
- Required -Degree in Nursing
- Required -Must possess sufficient prior work experience in progressively responsible positions during which the skills and abilities to perform all the essential functions have been amply demonstrated at a consistently competent level.
- Preferred -Experience in a community health center and working within a managed care environment.
The Call Center Triage Nurse provides front-line patient support by managing incoming calls, scheduling appointments, updating patient records, and ensuring exceptional customer service. Performs clinical triage by assessing patient concerns, providing nursing guidance, coordinating urgent care needs, communicating with providers and community healthcare partners, and documenting patient interactions accurately. Serves as a clinical resource to call center staff while supporting efficient, timely, and patient-centered care delivery.
Skills:
Requires strong knowledge of community healthcare, nursing operations, clinical management, and regulatory compliance. Must demonstrate leadership, customer service, and interpersonal skills, with the ability to coordinate across departments, manage multiple priorities, and provide care and education across all age groups. Proficiency in computer systems, documentation, and organizational skills is essential.
