The RN in this role completes in-home visits by talking with clients and families, review health history, and develop clear, practical care plans for non-medical in-home caregivers to follow. The work is focused on assessment, communication, and care planningnot clinical tasks.

What you will be doing
- Travel to clients homes to complete scheduled, non-clinical nursing assessments
- Conduct verbal and observational assessments with clients and family members
- Review medical history, diagnoses, medications, functional abilities, and daily routines
- Identify risks, support needs, and appropriate non-medical services
- Develop and update individualized care plans for in-home caregivers to follow
- Serve as a clinical resource to the care team regarding the clients plan of care
- Maintain accurate and timely documentation of assessments and care plans

- Active Registered Nurse (RN) license

This role includes traveling to clients homes for scheduled assessment visits and collaborating with the office team to support care delivery. There is no direct caregiving.
