We are seeking an LPN Care Manager to support outpatient clinic operations with a focus on care coordination, case management, population health, chronic disease management, and transitions of care. This role partners with providers to improve patient outcomes, reduce readmissions, and support value-based care initiatives.

ResponsibilitiesEssential Duties and Responsibilities (includes, but is not limited to):
- Partner with interdisciplinary teams to achieve care coordination, quality, and financial performance objectives.
- Lead and manage in-person and virtual meetings with practice managers, staff, providers, and care managers to share results and identify improvement opportunities.
- Stay current on payor and regulatory requirements and communicate updates effectively.
- Manage high-risk patients and support chronic disease management (diabetes, CHF, COPD)
- Document in EMR systems and support quality/HEDIS measures
- Provide patient education and remove barriers to care
Coordinate patient care plans, referrals, and follow-ups
Before we go any further, we do have some deal-breakers. You must have:
- Current and valid state license as an LPN.
- Current BLS (AHA) certificate upon hire and maintain current.
- A minimum of one year of experience in an applicable setting is preferred.
- IV certification upon hire.
