RN - Field Supervisor ( non medical home care )

Amazing Grace is looking for a loving and caring Registered nurse to assist with non skilled in-home care patient admissions, recertification and caregiver training. The nurse will be responsible for creating patient care plans, communication with doctors, family and case managers to fulfill the delivery of a patient centered plan of care. We currently have an open Position for a Registered Nurse (R.N.) with an active License and looking for full time position.

Job description:

Perform comprehensive, initial assessments of patients prior to the provision of care and initiate the plan of care.

Provide overall case management duties that include appropriate follow - up care and reassessments.

evaluating client’s care needs ongoing and applying necessary changes to plan of care.

Competency evaluations of caregivers

> Communicate with patient care team, including doctors, case managers and family members on a regular basis to ensure proper delivery of care.

Job Requirements:

Must be a licensed Registered Nurse (RN) in the state of Ohio.

A minimum of one year experience as a Registered Nurse is required

Previous experience in the home care setting is desired

Knowledge of home health regulations is preferred

Reliable transportation required.
