Fusion Medical Staffing is seeking a skilled Hospice LPN for a 14 week travel assignment in Elizabethtown, KY. Required Qualifications:
  • One year of recent experience as an LPN, including a minimum of six months in a Hospice Setting as an LPN
  • Valid LPN license in compliance with state regulations
  • BLS certification (AHA/ARC)
Preferred Qualifications:
  • Other certifications and licenses may be required for this position
Summary:

A Hospice Licensed Practical Nurse (LPN) provides hands-on nursing care to patients receiving end-of-life support, ensuring comfort, dignity, and symptom management. Hospice LPNs follow established care plans, administer medications, monitor patients for changes, and provide emotional support to families, all under the direction of an RN or physician. Their role is essential in helping patients experience a peaceful and comfortable transition.

Essential Work Functions:
  • Provide direct patient care according to hospice care plans, ensuring comfort and symptom relief
  • Administer prescribed medications and treatments, including pain management interventions
  • Assist in educating families and caregivers about hospice care, symptom management, and comfort measures
  • Observe and report changes in patient condition to the RN or hospice team for timely adjustments in care
  • Work closely with hospice nurses, social workers, chaplains, and other team members to ensure holistic care
  • Maintain accurate and timely documentation of all care provided, symptom changes, and patient needs
  • Ensure adherence to hospice best practices, safety protocols, and regulatory requirements
  • Offer emotional and psychological support to both patients and their loved ones during the end-of-life journey
  • Communicate regularly with the hospice care team to ensure continuous, compassionate care
  • Strong ability to complete charting and documentation requirements while meeting visit and productivity goals
  • Adhere to infection control, safety, and healthcare regulations
  • Perform other duties as assigned within the scope of practice
Required Essential Skills:
  • Critical thinking, service excellence and good interpersonal communication skills
  • The ability to read, write, & communicate in the English language is required
  • Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills
  • Physical Abilities - Remain in a stationary position, move about, move equipment (50-100lbs), pushing, pulling, bending
  • Interpersonal Skills - To work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalism
  • Technical/Motor Skills - Ability to grasp, fine manipulation, pushing/pulling and able to move about when assisting with procedures and/or using department equipment.
  • Mental Requirements - Must be able to cope with frequent contact with the general public, customers, and meeting deadlines under pressure. Must be able to work under close supervision occasionally as well as working without assistance from other personnel. Must be able to contend with irregular activity schedules occasionally and continuous concentration to detail
  • Sensory - Visual acuity, ability to effectively communicate.
