FULL TIME
DAY SHIFT
NO WEEKENDS

This position shall be responsible to organize, implement, and coordinate the daily operations of Adult Day Health Services in accordance with the registrants' needs and the New York State Department of Health.
Key Responsibilities
•Planning the adult day services program to meet individual needs of its registrants.
•Liaison with community agencies to maintain referral sources both to and from the program.
•Oversee case management services to registrants and their families when necessary.
•Facilitating and organizing advocacy efforts.
•Coordinating the development and ongoing review and monitoring of each registrant's individual plan of care, and making necessary program adjustment.
•Establish, maintain, and monitor the internal management systems to facilitate scheduling and coordination of services, and for the collection of pertinent registrant data.
•General supervision of program staff, volunteers, and contractors.
•Program activities to work effectively with the day services program.
•Monitor registrant's attendance, mood behavior, and degree of involvement so that registrant's progress or decline is evaluated, by noting, reporting and charting.
•Monitor and facilitate needed registrant referrals to community and in facility ancillary services.
•Monitor medication management of the registrants.
•Develop and maintain a good working rapport with other departments within Folts Homes organization to assure that daily activities can be performed without interruption.
•Coordinate activities with other departments.
•Involve registrants and family members in plan of care when possible.
•Monitor, document, and recommend to your supervisor the equipment and supply needs of the program.
•Monitor budget and financials for the department.

  • Attend and participate in professional adult day service in-service training as deemed appropriate.
  • Oversee the registrants engaged in scheduled programs to promote their physical, social, and mental well-being by applying activity experience and knowledge of the diagnosis and conditions of each registrant.
  • Maintain the confidentiality of all participant care information.
  • Perform any other pertinent duties as assigned by my supervisor.
  • I understand that emergency conditions may require me to temporarily work shifts other than the one for which I have applied and been assigned, and agree to such scheduling change or overtime when necessary to ensure proper operation of the Homes as directed by my supervisor.

Skills, Knowledge and Expertise
Must have a minimum of 3 years clinical experience, and a current New York State Registered Nurse License.
A Bachelor of Science in Nursing, and supervisory experience are preferred.
