Care Manager (LPN) - Transitions of Care

Location: Orlando, FL 32819

Schedule: Monday-Friday, 8:00 AM - 5:00 PM

Dress Code: Navy & Gray Scrubs

About the Role:

We're seeking a compassionate and detail-oriented LPN to join our Transitions of Care team. You'll play a key role in coordinating patient care, ensuring smooth transitions across care settings, and supporting patients and families through education and advocacy.

Key Responsibilities:

* Deliver exceptional customer service and patient-centered care

* Coordinate care plans with physicians and interdisciplinary teams

* Monitor high-risk patients and support discharge planning

* Educate patients and families on treatment plans and resources

* Document care activities in EHR and maintain HIPAA compliance

* Use motivational interviewing to promote patient self-management

* Collaborate with providers to ensure continuity and quality of care

Required Qualifications: * High school diploma or equivalent * Active Florida LPN (with IV Certification) or RN license * BLS/CPR certification (or obtain within 90 days) * Experience in acute clinical care, care management, or discharge planning * HMO experience * Bilingual (English/Spanish) - preferred
