Home Health License Practical Nurse (LPN) Contract Job in Dublin

Schedule: Monday to Friday, Full Time (on-call possible)

We are actively hiring an experienced Home Health LPN for a 13-week contract in Dublin, providing skilled nursing visits to geriatric patients in their homes. This is a great opportunity to join a compassionate team while enjoying competitive pay and benefits.

• Active LPN license

• Minimum 1 year of geriatric home health experience

• Experience conducting skilled nursing visits in patients homes

• Proficient with HomeCare HomeBase (HCHB) documentation

• Reliable transportation and valid driver's license/insurance

• Strong clinical judgment and communication skills

• Conducting in-home skilled nursing visits (wound care, med management, patient education, etc.)

• Documenting care using HomeCare HomeBase (HCHB)

• Collaborating with care teams to ensure quality outcomes

• Monitoring patient condition and reporting changes

• Providing compassionate, patient-centered care to seniors

• Contract flexibility with possible extensions
