Licensed Practical Nurse Home Health Contract Dublin

We are seeking a Licensed Practical Nurse (LPN) with recent geriatric home health experience for a contract assignment in Dublin. This position involves performing skilled nursing visits to patients in their homes, and requires prior experience using HomeCare HomeBase (HCHB) electronic documentation.

Key Qualifications:

• Active LPN license

• Minimum of one year of recent geriatric home health experience

• Skilled in providing home-based nursing care (e.g., wound care, medication administration, assessments)

• Proficient with HomeCare HomeBase (HCHB)

• Reliable transportation and valid drivers license

• Strong clinical, communication, and time management skills

Responsibilities:

• Travel to patient homes throughout surrounding areas

• Deliver skilled nursing care, including wound care, injections, patient education, and chronic condition management

• Accurately document care and visits using HomeCare HomeBase

• Monitor patient conditions and report changes to the supervising RN or clinical manager

• Collaborate with interdisciplinary team members to ensure high-quality, coordinated patient care
