AuthoraCare Collective is currently seeking an LPN Patient Services Coordinator for our Integrated Health Services (Skilled home-based-care) department supporting our High Point, Kernersville and Winston-Salem service areas (office base - High Point) . This is a full-time in office position, Monday - Friday 8:00am - 5:00pm.

The Integrated Health Scheduler/LPN works directly with Integrated Health clinical manager and director to provide high quality clinical support to the Integrated Health patients through scheduling patients, navigating the EMR systems, checking in patients via EMR, updating provider calendars and schedules, telephonic or telehealth support, verify continued care as necessary, and following up with patients as requested in a clinic/community-based setting.

Responsible for completion of intake and setting the patient up in EMR. LPN will identify incomplete or noncompliant referrals and work to resolve any missing information. Communicate effectively both verbally and in writing with the Integrated Health Team, and community physicians. Identify patient needs in the home or facility and coordinate accordingly. Document the outcome of care in the appropriate EMR system. Requirements

Education and Experience :

  • Must be a graduate of an accredited LPN program.
  • Must have a current North Carolina LPN license.
  • One year's LPN experience is necessary, preferably two years in an acute care, hospice, LTC or home health setting.
  • CPR Certification
  • Ability to establish and maintain effective working relationships with patients, other employees, and the public.
  • Microsoft Word, Excel and HCHB experience strongly preferred.
  • Knowledgeable about medical terminology.
  • Flexibility in a demanding work environment.
  • Must have excellent customer service skills.
  • Must have a team mentality, flexibility, and willingness to learn
  • The ability to communicate effectively, multi-task and work under pressure is the key to success in this position, which also requires attention to detail and a commitment to achieving consistent customer satisfaction.

Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
