Job Summary and Qualifications

The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift. In collaboration with other members of the management team, the CNC directs, monitors, and evaluates nursing care in accordance with established policies/procedures, serves as a resource person for staff, and models a commitment to the organization's vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance.

What you will do in this role:

Assists with admission and discharge processes to ensure efficient throughput and high quality, patient-centered care.

Participates in the ongoing assessment of the quality of patient care services provided in the unit, in collaboration with other members of the management team.

Collaborates with subject matter experts and other managers to create an environment of teamwork that supports improved outcomes and service.

Supports a patient-first philosophy and engages in service recovery when necessary.

Supports the efforts of the facility to improve engagement by operationalizing current nursing strategies, including employee rounding, hourly rounds, and other initiatives.

Provides recommendations related to interviewing, selecting, and training new staff. Recommends and implements courses of action, including training and development, conflict resolution, personnel policy compliance, completion of performance evaluations, and/or disciplinary actions to ensure a competitively better organization.

Assists with staff scheduling. Manages all practical aspects of staff labor in accordance with patient care needs and established productivity guidelines.

Supports proper inventory control and assists with managing supplies and equipment.

Position Summary:

Under the general supervision of the CMO, the Palliative Care CNC supports the mission of Presbyterian St Luke's Medical Center of optimizing quality of life of all those served by expanding beyond the traditional focus of medical needs to the needs of the entire person. This position requires a nurse leader, expert in palliative care. The Palliative Care Nurse exemplifies professional nursing practice through education, consultation, and research activities. The role requires collaboration with ancillary teams within the hospital including chaplaincy, therapies, case management and clinical ethics team to support the needs of patients and their families. The Palliative Care Nurse will utilize assessment skills to identify symptoms, pain levels and overall well-being of patients while demonstrating clinical proficiency in pain and symptom management in collaboration with the Palliative Care provider. The Palliative Care nurse will coordinate, participate in and lead patient/family meetings to facilitate communication regarding (but not limited to): goals of care, understanding of treatment options and providing education on outpatient palliative care and hospice. The Palliative Care nurse will collect and manage data to support program management and growth.

What qualifications you will need: 5 years of nursing experience; previous experience in hopsice or palliative care required with additional experience in oncology and/or ICU preferred

Current RN licensure in the state of Colorado, or current active multistate RN licensure

BSN strongly preferred

(CHPN) Certified Hospice and Palliative Nurse, or (OCN) Oncology Certified Nurse must be obtained within 1 year of employment start date

Basic Cardiac Life Support must be obtained within 30 days of employment start date

Continuing demonstration of clinical and leadership competency. Must have the ability to assess information, apply critical thinking processes to determine course of action and relay this information to the interdisciplinary treatment team. Requires excellent interpersonal skills, conflict management and negotiation abilities and strong written and verbal skills. Must be able to prioritize workload and manage both time and stress effectively. This individual will need the ability to diplomatically interact with patients, family members and other professionals and document these interactions in a clinically precise and timely manner.

Demonstrates good organizational skills. Provides safe and appropriate patient care within the standards of nursing practice. Displays judgment and superior problem solving skills. Communicates positively and professionally with all internal and external customers.
