With all of the health risks our elderly and health care workers are facing, the home is one of the safest settings for both our clients and our support staff. 

We are growing and looking to add a new role to our team for Nurse Supervisor/Client Care Manager to help implement and integrate our industry changing “8 Care Needs Program”TM to help our elderly clients age in place (in their own home) while receiving TOP NOTCH care led by you! 

We are looking for someone caring and compassionate and also committed to quality care for our seniors.  

In this Role You will be responsible for:

  • Meeting with potential clients and family members to discuss their needs and provide solutions while helping them see the value Home Instead and our 8 Care Needs Program offers.
  • Creating their “CARE PLAN” in alignment with our 8 CARE NEEDS wellness program
  • Orienting our CARE TEAM to the client and their care needs as well as Supervising and training CAREGiving field staff.
  • Maintaining on-going communication and a consistent follow-up with potential clients and families. 
  • Work with other team members to coordinate various aspects of a client’s care.
  • Conduct client/CAREGiver introductions with every new client and with every new CAREGiver and assist with other client/CAREGiver Introductions as needed.
  • Create and maintain client and responsible party records documenting all communication and quality assurance meetings/visits.
  • Modifying service CARE Plans as necessary
  • Reporting and managing Weekly Care Critical Numbers
  • Participate in On-call rotation
  • Effectively handling client “Care Concerns” and making modifications as necessary

WHAT WE ARE LOOKING FOR:

  • College degree preferred, RN licensure
  • One year of experience in home care, health care or senior-related industry preferred, an equivalent combination of education and work experience may be considered.
  • Must possess a valid driver’s license
  • Someone Caring & Compassionate with good Bedside Manner
  • A Nurse Who Cares Deeply About Quality
  • The Ability to “connect” with others
  • A great Team Player---but also able to work independently
  • Great COMMUNICATION skills – a MUST
  • Someone who wants to Grow Personally & Professionally
  • Basic Computer skills with some proficiency in WORD & Excel
  • Ability to conduct occasional assessments on the weekends or evenings as necessary

Salary range for this position is $55K-$65K (or hourly equivalent) with bonus potential; 32 hours/week
