Description

+ Ensuring that the clients plan of care drives all service activities

+ Ensuring the safety of the clients while in their home and community

+ Assisting clients in exploring and pursuing areas of interest in employment by accessing community activities and resources for Community Support and Community Membership only

+ Ensuring Community Membership services are provided in the clients community and are centered around the clients individual interests and plan of care

+ Performing other duties as assigned

Qualifications

+ Proof of vehicle insurance at $50K/$100K and a valid Maine drivers license

+ Must be 17 years of age or older

+ High school diploma or equivalent preferred

+ DSP certification

+ Entry level computer experience and must have access to email/internet

+ Mileage reimbursement
