Position Overview

A healthcare organization in the Bronx is seeking an experienced Care Delivery Manager (RN) to support clinical operations, lead care coordination efforts, and uphold high standards of patient care delivery across medically complex populations.

Key Responsibilities

• Oversee the development and execution of individualized care plans to improve patient outcomes
• Lead and participate in interdisciplinary team (IDT) meetings and care coordination initiatives
• Supervise clinical staff, including coaching, mentoring, training, and performance management
• Ensure appropriate staffing coverage and support efficient day-to-day clinical operations
• Monitor quality of care, documentation accuracy, and adherence to regulatory and clinical standards
• Review clinical assessments and collaborate with clinicians to ensure accuracy and completeness
• Coordinate transitions of care across hospitals, rehabilitation facilities, home care, and community-based settings
• Investigate incidents and ensure timely follow-up, resolution, and documentation
• Analyze clinical delivery reports and support quality improvement initiatives and process enhancements
• Assist with onboarding, mentorship, and ongoing professional development of team members

Qualifications

• Active New York State Registered Nurse (RN) license required
• BSN required (or equivalent healthcare leadership experience in a healthcare setting)
• Managed Long Term Care (MLTC) experience required
• Minimum 2+ years of management or supervisory experience in healthcare required
• Strong background in community health and/or homecare required
• Experience working with elderly or medically complex populations preferred
• Knowledge of care coordination, clinical best practices, and regulatory compliance standards
• Strong leadership, communication, organization, and interdisciplinary collaboration skills
