Schedule:

• Full-time, Monday through Friday
• On-call responsibilities may be required

Required Qualifications:

• Active RN license
• At least 1 year of recent home health RN experience
• Proficient with OASIS documentation
• Experience with HomeCare HomeBase strongly preferred
• Reliable transportation and a valid driver's license
• Ability to manage a consistent daily caseload

Key Responsibilities:

• Deliver in-home nursing care, skilled clinical interventions, and patient education with a focus on individualized treatment plans
• Complete thorough patient assessments, ensure OASIS documentation accuracy, and submit records promptly
• Collaborate with physicians and interdisciplinary care teams to optimize patient outcomes
• Manage wound care, medication administration, and chronic disease monitoring
• Communicate changes in patient condition and progress updates to team members
