Schedule:

• 13-week assignment with opportunities to extend
• Full-time, Monday through Friday
• Daily visit requirements with potential for on-call responsibilities

Primary Responsibilities:

• Deliver skilled nursing care and interventions to patients in their residences
• Complete thorough patient assessments and timely OASIS documentation
• Collaborate with physicians and interdisciplinary care teams to implement personalized care plans
• Perform wound care, medication administration, patient and family education, and monitor disease progression
• Communicate relevant updates or condition changes to the clinical team promptly

Required Qualifications:

• Active RN license
• Minimum of one year of recent home health RN experience
• Demonstrated OASIS proficiency
• HomeCare HomeBase experience strongly preferred
• Reliable transportation and valid drivers license
• Commitment to a full-time, structured schedule and quality patient care
