RN Home Health - Gray, LA

We are seeking a dedicated Registered Nurse (RN) to provide skilled home health services on a full-time, 13-week contract. This position is ideal for someone with OASIS experience, exceptional clinical judgment, and confidence managing a schedule of daily visits.

Schedule:

• Full-time, Monday through Friday
• On-call responsibilities may be required

Key Responsibilities:

• Deliver evidence-based nursing care and interventions directly in patient homes
• Perform comprehensive assessments and complete OASIS documentation accurately and promptly
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans
• Conduct wound care, medication administration, patient education, and ongoing disease management
• Communicate effectively with the clinical team regarding patient status and care updates

Qualifications:

• Active RN license in good standing
• At least one year of recent experience as a home health RN
• Strong proficiency in OASIS documentation (required)
• Familiarity with HomeCare HomeBase is highly preferred
• Ability to independently manage a full daily patient caseload
• Reliable transportation and a valid drivers license
