Job Description
The Hospital Liaison Nurse LPN coordinates and facilitates the delivery of homecare services in conjunction with the homecare team, hospital discharge planning staff and other referral sources. The Hospital Liaison LPN facilitates referrals, coordinates skilled and non-skilled services, and other community resources through collection and input of data, interview and team collaboration in a variety of settings. The Hospital Liaison Nurse LPN performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse.
Education And CredentialsHS - High School Diploma or GED required. AAS - Associate of Applied Science LPN preferred.NYS Drivers License upon hire required.
Experience
1 year of experience in medical/surgical nursing required. 1 year of experience in Homecare setting preferred. Licensed Practical Nurse upon hire required.
