Multi Specialty LPN - Full Time - Days -(UTICA, NY)

The Speciy Licensed Practical Nurse provides general nursing care to pients, families and caregivers in the Medical Group practices. The Licensed Practical Nurse assists and advoces for the pient and families health in support of the pient centered approach and delivery model of care, and is an active care team member. Responsible for collaboring with physicians/ providers , registered professional nurses and multidisciplinary care team members. A Licensed Practical Nurse can perform tasks and responsibilities within the framework of case finding, health coaching, and supportive care under the direction of the registered professional nurse or licensed physician or licensed health care provider. Nursing shall perform in accordance with the standards outlined for Licensed Practical Nurse in the NYS Educion Law, NYS Office of the Professions, and by the Ste Nurse Practice Act. A Licensed Practical Nurse will follow the mission, vision, I-CARE values, policies and procedures of Mohawk Valley Health System.

• Perform tasks within the framework of supportive care, under the clinical direction of licensed health care provider or registered professional nurse
• Support self-care , self- management and the established plan of care.
• Utilize standard precautions and practice all times
• Maintain equipment, supplies, inventory, and a safe and clean work environment
• Maintain technical knowledge by tending educional workshops, mandory in-services; reviewing publicions and participing in department committee meetings or work groups
• Engage in quality improvement initiives including continuing educion, participion in system-wide and department specific teams
• Maintain a cooperive relionship among the healthcare team by effectively communicing informion
• Respond to pient requests, communices effectively to the licensed health care provider who can fulfill the pient request

Required:

• Gradue of an accredited school of licensed practical nursing program

Preferred:

• Bachelors or Masters of Science in Nursing from accredited nursing program

Required:

• Current New York Ste licensure as a Licensed Practical Nurse
• Basic Life Support Certificion - Must receive BLS certificion within 6 weeks of hire de
