Job Description

Under provider direction, the RN provides skilled nursing care to pients in a variety of clinical settings. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evalue human responses to actual or potential health problems utilizing approprie practices, standards, protocols and guidelines. This position reports to a Practice Manager.

• Provides office-based nursing care in collaborion with provider
• Assesses physical and psychosocial needs and environmental factors affecting pients and their families, formules nursing plan of care and ensures continuity of pient care through approprie nursing evaluion and intervention
• Communices with provider regarding pient needs, nursing assessments, and recommendions
• Takes direction and inities actions (cross/additional training) th will allow seamless pient care
• Collects and documents current health stus and medical history and assists in scheduling pient office visits
• Reviews pient records, including preoperive records, to ensure completeness and reviews outstanding issues with provider
• Demonstres independent nursing actions based on assessment and problem identificion
• Under direction of provider, calls in prescription medicions to pharmacies and observes and documents pients response to pertinent medicions
• Documents accure and ongoing assessment of pients needs, problems, capabilities, limitions, progress toward goals, and response to care
• Triages telephone calls and displays ability to initie approprie nursing intervention in prompt, precise, and professional manner
• Assists pients and family members or other clients with concern and emphy; respects confidentiality and privacy

Travel to sellite locions as needed

• Current Connecticut RN License required.
