Make a meaningful impact by providing compassionate, patient-centered care for individuals during their end-of-life journey. In this field-based contract role, you will be entrusted with delivering skilled hospice nursing care and emotional support in patients' homes and residential settings. Your expertise and empathy will ensure comfort, dignity, and peace for both patients and their loved ones.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional weekend or on-call rotation as needed
  • 13-week contract commitment

Key Responsibilities:

  • Conduct comprehensive nursing assessments and develop individualized hospice care plans.
  • Provide skilled nursing interventions focusing on symptom management, medication administration, and patient comfort.
  • Collaborate with interdisciplinary team members, including social workers, chaplains, and aides, to address holistic needs.
  • Monitor changes in patient conditions and promptly report to the hospice medical director and care team.
  • Educate patients, families, and caregivers on disease progression, comfort measures, and emotional support strategies.
  • Maintain clear, accurate, and timely documentation in Home Care Home Base (HCHB).
  • Participate in case conferences, team meetings, and quality improvement initiatives.
  • Ensure compliance with hospice protocols, procedures, and regulatory guidelines.

Qualifications:

  • Active RN license in good standing in the state of practice
  • Minimum of one year of recent hospice nursing experience
  • Proficiency in Home Care Home Base (HCHB) electronic documentation
  • Strong clinical assessment, communication, and independent fieldwork skills
  • Deep understanding of pain management, comfort care, and family support
  • Reliable personal transportation, valid drivers license, and auto insurance
  • Compassionate, resilient, and empathetic approach to patient care
  • BSN preferred; prior palliative or end-of-life care experience is a plus
