Hospice Home Health Registered Nurse in Madera, CA

Make a difference each day as a compassionate and skilled Registered Nurse dedicated to providing hospice care. This field-based position involves supporting patients and families during end-of-life transitions, offering both expert clinical guidance and emotional support.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional weekend or on-call rotation may be required

Qualifications:

• Active RN license, in good standing
• A minimum of 1 year of recent hospice experience
• Proficiency with the Home Care Home Base (HCHB) electronic documentation system
• Strong clinical assessment and communication skills
• Excellent understanding of symptom management, end-of-life care, and family-centered support
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to work independently with a flexible, field-based schedule
• Compassionate and emotionally resilient nature

Preferred:

• Bachelor of Science in Nursing (BSN)
• Experience in palliative care or similar environments
• Knowledge of Medicare hospice standards and documentation

Key Responsibilities:

• Conduct initial and ongoing nursing assessments to develop personalized care plans
• Deliver direct patient care focused on comfort, dignity, and symptom management
• Administer medications and treatments according to physician orders and hospice protocols
• Provide thorough and supportive education to patients, families, and caregivers
• Collaborate with an interdisciplinary team to address emotional, spiritual, and physical needs
• Accurately document all care provided using HCHB system
• Attend case conferences and participate in ongoing quality initiatives
• Ensure compliance with hospice policies and regulatory requirements
