Rogue Community Health is looking for an RN Nurse Case Manager for our clinics who is not only committed to exceptional patient care but equally interested in supporting staff within a cohesive team environment. We are currently hiring for our Eagle Point and Butte Falls Health Centers. This position will ensure the efficient and effective flow of patients during scheduled clinics. Within the RN scope of practice, utilize the nursing process to provide patient care and education which will focus on patient engagement and behavioral change, treatment compliance, and health risk factor reduction. This position will work collaboratively through joint planning and problem-solving with clinic staff and improve the management and stability of medically complex patients resulting in reduced ER visits, hospitalizations, treatment-related complications, and improved patient quality of life.

ESSENTIAL RESPONSIBILITIES ADDENDUM FOR NURSE CASE MANAGER:

  1. Coordinate the care essential to the assessment, promotion, maintenance and restoration of health and wellness for patients with exceptional care needs, defined as patients with the following diagnosis: Chronic disease, substance abuse, DSM V or chronic pain, all insulin dependent diabetics and patients who require care management between multiple care management team members

  2. Screen and enroll patients in the Exceptional Needs Case Management Program, ensure appropriate documentation and tracking of patient goals and outcomes

  3. Promote patient engagement and behavioral change with patient education through, planned, regular interactions with the patient and family/caregivers

  4. Plan with the patient, family, and care team for care that is feasible and within the physical, financial and emotional resources of the individual including addressing barriers when the patient has not met treatment goals

  5. Develop, implement, evaluate and communicate to the patient/family and care team members the plan of care for managing the patient's needs

  6. Facilitate referrals to specialist and community resources needed to manage the patient's care

  7. Coordinate responses to patient needs and facilitate communication between patient, care team and stakeholders

  8. Interpret to the patient and family diagnoses and treatment plans

  9. Teach basic principles and assist patients in managing their chronic diseases through setting self-management goals, self-efficacy and behavior change

  10. Respond to phone calls and MyChart communications from patients, triage requests and respond according to need

  11. Schedule qualified patients for appointments based on established protocol, identified need, and availability of appointments including following up with patients/families who have not kept important appointments

  12. Maintain accurate and legible records of interventions

  13. Participate in monthly nurse meetings to ensure quality patient care and improved patient outcomes Participate in projects and initiatives as assigned by supervisor
